Our duties
GWB Pharmacy LLC is a covered entity under the federal Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy and security of your Protected Health Information, to give you this Notice of our legal duties and privacy practices, to follow the terms of the Notice currently in effect, and to notify you in the event of a breach of unsecured PHI.
How we use and disclose your PHI
We may use and disclose PHI without your specific authorization for the following purposes:
- Treatment. To dispense prescriptions, counsel on use, identify interactions, and coordinate care with your prescriber.
- Payment. To bill and collect payment from you, your insurance plan, or a third-party payor.
- Health care operations. Quality assessment, staff training, accreditation, licensing, and routine business activities.
- As required by law. Including reporting to public health authorities, the FDA, or in response to a valid court order or subpoena.
- Appointment reminders and refill notices. Through the contact channel you have selected.
Other uses and disclosures will be made only with your written authorization, which you may revoke in writing at any time.
Your rights
- Access. Request and inspect your PHI; request a copy in a designated record set.
- Amend. Request a correction to PHI you believe is inaccurate or incomplete.
- Accounting of disclosures. Request a list of certain disclosures we have made of your PHI.
- Restrict. Request that we restrict certain uses and disclosures. We will accommodate reasonable requests when required by law.
- Confidential communications. Request that we communicate with you by alternative means or at an alternative location.
- Paper copy. Request a paper copy of this Notice at any time.
- File a complaint. File a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate.

Changes to this Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have, as well as for PHI we receive in the future. The current Notice is posted in the pharmacy and on this page. The effective date is shown at the top.
Complaints to HHS
You may file a complaint with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, or by visiting hhs.gov/hipaa/filing-a-complaint.
